Imagine a hospital, a place where we expect to find stability and expertise, yet one in six consultants across Ireland are working on temporary contracts. This revelation, presented to the Oireachtas Health Committee by the HSE, raises some crucial questions.
The Locum Consultant Conundrum
A total of 763 consultants, or 16.5% of the total, are employed on non-permanent terms. These locum consultants, as they're known, are doctors hired for fixed or short-term periods, often to plug gaps in staffing. While they're a vital part of keeping services running, over-reliance on locums can make long-term planning a challenge.
The data shows that general medicine has the highest number of locum consultants (236), followed by psychiatry (141) and surgery (101). Smaller hospitals, like those in Tralee, Letterkenny, Tullamore, and Tipperary, are particularly reliant on locum staff, with numbers ranging from 15 to 32.
But here's where it gets controversial: Fine Gael TD Colm Burke, a member of the Oireachtas Health Committee, argues that this heavy reliance on locums is hindering long-term planning across the health service. Burke highlights that consultant posts are often only advertised after a doctor has retired, even when management had prior knowledge of their departure.
"It's hard to see how a consultant, with no prospect of permanency, can develop and lead a team effectively," Burke says. He adds that research from the UK suggests that over-reliance on locums can lead to quality and safety concerns if not addressed properly.
Burke also shares concerns raised by permanent consultants about the impact of high locum usage on continuity of care, team morale, and patient communication.
"With a new CEO starting soon, now is the perfect opportunity for the HSE to set clear targets for 2026 to fill vacant posts and reduce the reliance on locum consultants in Irish hospitals," Burke urges.
